In order to maximize positive outcomes for students as they graduate from high school, Esperanza’s Transition Coordinator is responsible for overseeing the preparations for adult programming and post-secondary enrollment.
Transition services include:
- Developing post-secondary goals for each student over 14 years of age by administering a career survey, a vocational assessment, and parent and student questionnaires.
- Coordinates connections and appointments with outside agencies that provide services to prepare students for employment or adult services.
- Information for parents about the options available for students after graduation and the process for acquiring these services. Also, provides parents with consultation on guardianship and Social Security.
- Works with Esperanza’s vocational staff to ensure that the instruction provided at Esperanza corresponds with the post-secondary goals for each student.
- Involves other programs of Esperanza Community Services that may be able to provide information or services to students and their families. These include the Client and Family Support Program and Coleridge Adult Training Program.
