Information on how to get state funding for your child
If someone in your family has a developmental disability, it is important to enroll that person in a system called Prioritization of Urgency of Need for Services (PUNS). PUNS is a program set up by the state to plan for present and future needs of people with developmental disabilities. In order for your family member to receive services from the state, it is necessary to fill out the PUNS form. Families can do this at a case management agency. If you live on the North Side of Chicago, you will need to contact Community Alternatives Unlimited (CAU), the case management agency serving this area. If you live on the South Side, please call Community Service Options (CSO).
